Hospitals & Primary Care Providers

The Florida Asthma Coalition collaborates with Hospitals and Providers throughout the state to decrease the burden of asthma. Our member’s engagement in the Hospital Workgroup and Provider Workgroup set the standards of asthma care, create recognition criteria for Asthma-Friendly Hospitals and Asthma-Friendly Providers, and stay abreast of best practices and changes to asthma care.

Asthma Action Plan

An asthma action plan is a written, individualized worksheet that shows patients and caretakers the steps to take to prevent asthma symptoms from getting worse. The National Asthma Education and Prevention Program’s Guidelines for the Diagnosis and Management of Asthma (EPR-3 Guidelines) recommend that healthcare providers caring for patients with asthma work with them to develop an asthma action plan.

The Florida Asthma Coalition recommends using the Allergy & Asthma Network’s Asthma Action Plan Template.

Asthma Friendly Recognition

Hospitals and Primary Care Providers throughout the state can earn ‘Asthma-Friendly’ recognition at any point during the year by meeting the following five criteria:

  1. Professional Development
  2. Use of Asthma Action Plans
  3. Providing Self-Management Education
  4. Providing Multi-Component Home Based Asthma Management

Hospitals that have already received recognition and are looking to reapply must meet the following criteria:

  1. Asthma Quality Improvement Process
  2. Tobacco Cessation Education
  3. Professional Development
